A 56-year –old woman presents to her doctor’s office complaining of gradually progressive, nonpainful enlargement of the terminal joint on her left hand over a 9-month period.She has some stiffness with typing but not first thing in the morning.She also reports pain in her right knee, which occasionally “locks up.” The right knee also hurts after long walks.On examination, her blood pressure is 130/85 mm Hg, heart rate 80 bpm, and weight 285 lb.Examination reveals only a nontender enlargement of her left distal interphalangeal (DIP) joint, and the right knee is noted to have crepitus and slightly decreased range of motion.There is no redness or swelling.

Normal Blood Pressure is 110-130mmHg systolic and 70-85mmHg diastolic. Hers is normal.
The diagnosis is a bit tricky enh! well to me it sounds like non inflamatory Osteoarthritis. The hints are the age of the pt, the gender, the duration of illness, the joints affected i.e. the distal joint and the knee joint -assymetrically, the pain in her knee after long walks and the crepitus.
I think they added in ‘no redness or swelling’ just to swoosh u away this diagnosis of OA since u would think OA is always inflammatory which is not always the case.
Initial treatment would be an initial trial of paracetamol. consider the addition of a topical NSAID, and then cpsaicin, for the knee and hand OA.
Dont forget to explain to the patient the full nature of the d’se, advice and instruct on appropriate exercise and reuce any adverse mechanical factors like weight!
